Evident Medical

For families whose loved one has developed psychosis, Evident brings specialist expertise, careful measurement, and practical support into one coordinated model for outpatient care.
OUR MISSION
What Makes Us Different
Evident is designed for families who need more than brief medication visits: focused expertise, careful monitoring, and a care plan that stays connected over time.
Psychosis-Focused Expertise
A specialized practice can go deeper on diagnosis, medication strategy, relapse prevention, and recovery planning.
Family‑Inclusive Care
When appropriate, we help families understand what is happening, what to watch for, and how to support recovery.
Measurement Guides Care
Symptoms, side effects, functioning, and safety are tracked over time so treatment decisions are not based on impressions alone.
Specialty Care From Home
Telehealth access across New York State, with the depth and continuity expected from specialized psychiatric care.
How We Help
We help clarify what is happening, reduce side-effect burden, and support a practical path forward.
How We Work
Our care model combines specialist assessment, measurement-based follow-up, family collaboration, and evidence-guided medication decisions.
Track What Matters
Symptoms, side effects, functioning, and goals are followed over time so progress is visible and decisions are grounded.
Structured Diagnosis
Structured diagnostic work helps clarify what is driving symptoms and creates a more accurate foundation for treatment.
Evidence-Based Decisions
Patients and families are guided through options with attention to effectiveness, tolerability, safety, and personal goals.
Telehealth and Remote Monitoring
Care can be delivered remotely when clinically appropriate, reducing travel burden while preserving continuity.
Recovery-Oriented Care
Treatment is anchored in the patient’s recovery goals, with education and support that extend beyond medication alone.
Peer and Practical Support
Support can include lived-experience perspective and practical guidance for challenges that go beyond prescriptions.
Our approach draws from coordinated specialty care, adapted to support patients across the full course of illness.
